As a licensed After School Care Provider of school-aged students, we are required to have certain information to comply with the Licensisng Standards.  Please ensure you read and fill out all forms clearly and completely, even if your child will be coming to one of our other Acting classes.

 

We know your time is valuable.  Although we do not currently have our forms as online documents, you may print them out here and either email them back to us at Gigi@ActingUpCDA.com, or bring them in when you drop by.  The Word document forms are editable, and the pdf forms are not.
